Ye, kena endocrine doctors yang jaga specialized medications macam CBZ dan PTU ni. Let your endocrine doc yg tukar to PTU, not just maner maner doctor aje.

Maaf doc saya tak paham 'Ada thread grave's disease; sy letak 2 minggu lepas ke apa' ? Sebelum nie saya tak pernah lakukan ape2 operation atau ambil RAI. Saya hanya bergantung pada ubat sahaja.

bila hyperthyroid, you will have signs and symptoms of higher metabolism:tachycardia, palpitations, sweaty palms, red palms, you would feel anxious and panicky; you hyeprvetilate, peolpe will see u as fidgety, restless and anxious or even hyperactive. you tendon reflexes are very brisk, you will lose weight, gets diarrhoea, always hungry; you will lose weight and your periods will be thin and infrequent; you can have itchy skin and lose your hair from your scalp; u commonly will have very fine tremors esp in your hands. you will feel tired. you bleh dapat goitre and if you have Grave's Disease, you will get bulging or staring eyes.

no, itchy skin in hyperthyroid ni biasanya takde rash; yg u described tu ialah rash.
makan ubat untuk hyperthyroidism maksud you (ie carbimazole?)? ye, ada side effects nya.
jika ada yperthyroidism tapi tak makan ubat, maka boleh dapat high-output heart failure.
hyperthyroidism ni memang kena treat betul betul.

ade iras iras tu bunyinyer.
kena buat lah Thyroid Function Tests (TFT).
buat jugak Fasting Blood Sugar (FBG) dan HbA1C... utk cek Diabetes (DM) kot kot ade. Ngomel: org di Malaysia ni penuh dgn diabetes, ngape la, tak reti langsung jaga pemakanan etc; tak tau ke hidup singkat dgn DM ni.

ha, memang ade TFT, FBG HbA1c kat clinic kerajaan pon; takyah gi swasta... nothing extra there apart from the bill. Also, suruh staff also check fasting cholesterol awak.. tu lah, ade masa ngomel nih; tgh oncall nih; bila oncall bila free jap bleh ha ngomel dan complain itu ini.

eg kena buat scope dulu tgk vocal cords sbb bila dah buat op nnt, vocal cords boleh lumpuh.
kena ubah regime carbimazole dulu sebelum op
jugak mungkin kena ambil lugol's iodine dulu pre-op.
radio-uptake scans kene buat dulu.
ngapa? u nak gi op ke?
if hypothyroid, lain sikit approach dia

so itchy skin tu tiba2 gatal tanpa sbb la cmtu..
yes carbimazole apa side effect nya? n lg satu propranolol pn ada side effect juga ke?

side effects CBMZ ialah nausea, stomach irritation, itching, dan a few others. tapi yg penting ialah satu rare side effects dia, ie agranulcytosis. ini ialah bila cells darah merah tetiba tak functions.. u akan dapat sore throat dan mula dapat fever dan jadi unwell. kejap je lepas tu bleh dapat infections yg kow kow nyer sebab dah takde darah putih; sure case nyer masuk ITU dan can be fatal. alhamdulillah agranulocytosis ni sgt jarang berlaku.
propanolol tu hanya utk kurangkan symptoms aje.. ie utk slow kan herat rates sikit, utk improve palpitations dan utk kurangkan tremor. propanolo tidak block thyroid hormones pon, CBMZ yg block.

maknanya carbimazole itulah effectnya.. smua doctor tau ke pasai side effect ni?
cmna plak klu sesorg tu ambl carbimazole tp hyperthyroid lg truk? atau carbimazole tu gagal brfungsi dkat badan hyperthyroid? |

kalau amik CBMZ, hyperthyroid pasti improve punye lah.
tapi kalau dose rendah, kena la doctor tu naikkan; eg jika 10mg BD tak berkesan, naik kan ke 20mg BD.. tapi kene la buat TFT dulu tiap kali; buat TFT ni 6 weeks after each time doctor naik kan dose tuh
doctors sepatutnya tau lah side effects ni. malah, docs wajib bagi tau side effects yg satu ni.. kat sini, u takleh start CBMZ ni selagi u tak bagitau dia this particular side effects walaupun rare. Rare tak rare, dah berapa kali saya jumpa ( tapi sy detect kerap sebab ini bidang sy dan pts yg dtg ramai yg on CBMZ ni)

eh kena intubate; kan op nyer kat leher; kena support la breathing; also urat darah besar besar ibu jari kat situ, dan berbaris baris nerves lalu kot leher tu, maka takle boleh sejam je op. thyroid tisu tu pon kena handle pelan pelan dan elok elok masa op.. terkeluar thyroid hormone byk byk masa op bleh mati pt tu nanti. kene surgeon yg biasa buat

Nak tanya how soon sepatutnya a newborn that detected as having conginetal hypothyroidism kena start treatment?
Baby baru 3 minggu now. Should the parent worried or not in case treatment start lambat akan effect kepada future baby? |

sepatutnya straight away; the aim is nak diagnose cepat cepat dan normalize kan TFT tu sebelum umur 3 minggu; yang penting ialah perkembangan otak tu dulu; ngapa? hopefully baby awak takde lah kene kes gini. di mesia lambat cam kot.. nanti baby tu dikata baby yang baik, tak nangis, tidur selalu.. walhal baby tu takde daya. harap baby awak sempurna takde apa apa masalah insyaAllah. tahniah atas ketibaan baby baru ya
