Allergic Ubat

Q: Soalan saya. Saya nak tahu petanda pesakit yang allergic antibiotik penicilin. Saya antara yang dikesan allergic pada ubat ni. Pernah saya rasa macam sesak nafas dan sakit dada dan lain-lain.
Bukan sekali saya pernah dapat penicilin, tapi kesan dia macam tu la setiap kali, dan saya terpaksa berhenti makan.

Saya ada pergi ke hospital, pihak farmasi nak bagi ubat dekat saya, tapi bila dia check, ubat tu 1 group dengan penicilin. Kemudian, mereka tukar. Dan berikan saya cephalexin. Kata mereka tu ibarat sepupu sepapat pencipin la. Hehe. Tapi dengan pesanan, andai kata saya rasa tak ok makan dalam sehari dua, saya datang ke hospital lagi, refer doctor n tukar ubat.
Masalahnya , saya takut nak makan. ;)

Apa patut saya buat ek?

A: Tanda2 allergic ni sama jika allergic pd ubat @ makanan. Badan akan naik ruam2 merah macam kelopak garam, gatal, jika tanda yg teruk adalah bengkak bibir, muka & sesak nafas. Jika berlaku begini, kena cepat pergi kecemasan.

...allergic reaction nie can present itself as mild...to moderate to severe reactions...common sign and symptoms of allergic reaction includes rashes...which can be generalised or localised to an area of the body...these rashes can sometimes appear to be raised and in patches (hives)...itchy also...some pple also have runny nose and teary eyes...periorbital swelling also is one of the signs of an allergic reaction...swollen lips...and in severe cases patient can have chest tightness and difficulty in breathing (anaphylactic reaction) which would require immediate medical attention...cos if treatment is not initiated fast it can be fatal.

...in your instance...u are allergic to penicillin and the reaction that u had was a severe one...hence...u have to avoid antibiotics that are derivatives of penicillin...such as amoxicillin...ampicillin...cloxacillin...augmentin (amoxicillin and clauvulanate)...

...cephalexin is a first-generation cephalosporin antibiotic...and yes...it has been said that theres about 10% risk of cross-reactivity in patients that are allergic to penicillin...however...according to some literature reviews...the actual risk is about 1% saja...


...however...kita pon kena amik into consideration factors such as the severity of your allergic reaction yang u previously had with penicillin before deciding to prescribe u with an antibiotic from the cephalosporin group nie...

...some doctors that i worked with would prescribe a lower dose of the antibiotic...eg...instead of 500mg of Cephalexin (2 capsules)...they give as 250mg of Cephalexin (1 capsule) first and patient to observe for any signs of allergic reaction...with the advice to discontinue the medication if pt develop reaction and to seek medical attention immediately in the event the patient had anaphylaxis...


...some also prescribe piriton (antihistamine) to these patients and advised them to take it together with their antibiotics...to minimise the allergic reaction...

...i can understand your fear to consume the cephalexin...but...if your condition requires the use of antibiotic as its primary treatment...then u need to take the medication...and complete its full course...kalau ko masih takut dan ragu2 nak makan...then please return to your doctor again for consult and let your doctor know about your concern nie...so that he can address your concern accordingly...
