masalah gusi tu juga kita kenali sebagai periodontal (gum) disease.

the responsibility of controlling the disease process rests completely with the PATIENT! This is not to say that professional care (i.e. diagnosis, education, therapy) is not necessary - it's just that MANY people that have had the best of care, do not perform the diligent home care regimen they should. Once a person has been diagnosed, they forever have periodontal disease. There is NO cure - it can only be maintained, and should be reevaluated at regular intervals so as not progress to the point of tooth loss. One of the biggest misconceptions concerning periodontal disease is that the dentist / hygienist will control the disease. Bacteria is the primary culprit.

periodontal treatment :-
1. scaling / root planning : SCALING consists of removing the hard calculus and plaque around the collar of the tooth as well as below the gumline. ROOT PLANING removes the calculus and plaque off the root surface well below the gumline (into the pocket). Depending on the case, these procedures may or may not require numbing. Hand instruments called scalers as well as high-frequency polishers will be used to accomplish a complete and thorough job

2. surgical treatment : When the pockets are very deep, procedures must be performed so that all infection, plaque and calculus may be removed. Sometimes the bone around is no longer smooth and it must be corrected. There are times that bone may be actually added to areas where it has eroded away

ni ada skit gambaran mengenai periodontal disease:-


this is the picture of healthy tissue. there is no bleeding or puffy gums (gusi), pockets all measure to a normal 3mm or less



this is the picture of periodontitis stage 1 (gingivitis.) bengkak & kemerahaan kelihatan pada gusi.. bleeding gums when measured, puffy in appearance and pockets no greater than 3mm. no damage to the supporting bone in this stage.



this is the picture of periodontitis stage 2. bleeding and puffy gums that measure slightly more than normal at up to 5mm



this is the picture of periodontitis stage 3. bleeding and swollen gums with pockets that will measure up to 6mm and more. recession beginning to appear.



this is x-ray picture of periodontitis. dapat diperhatikan level tulang semakin menurun & berlaku kehilangan tulang di antara akar gigi molar (geraham)

puteri baru je buang gigi bungsu yang 'jahat' itu kelmarin. mula mula pegi klienik, klinik refer kat sepital. 2 jam dok buat benda tuh.. aduiii.... sbb gigi putreri tu facing pipi dan tak nampak, so prosidur dia susah... abis semua 'ragum' keluar dari laci nak cari paling kecik sekali..... las las terkeluar laa.. tapi punya laa seksa.. 6 kali di bius... sbb masa dia cabut tu tetibe je sakit... terasa urat kat gusi tu tertarik, di injet lagi... sampai laa rehat, sambung rehat sambung.... sbb kat hujung langit langit...(nom. 28 kot sbb doktor asik sebut 28 je) bius tak kene... bila dah tercabut, terasngkut lak kat gigi sebelah... aduiiii....... menangis puteri... misi tu suh doktor tu belah je.. tapi doktor tu baik.. tak pe dia kata, pastu alhamdulillah... tertanggal (tensen gak ngan misi tu.. dia lak pepandai suh doktor tu belah laa... tarik je laaa)... dah laa masa x-tray sampai puteri terbelahak 2 kali nak masukkan wayar tu dalam mulut.... then the doctor tu discover satu lagi problem baru!!!!!! Arghhhhhhhhhhhhhhhh ada lagi satu gigi geraham yg tak keluar sedang terbaring (namapak kat x-tray).. dan dia kaata yg tu kene buang kalau tak sakit... waduhhhhhhhhhh...

ni masih sakit lagi...doctor tak kasi antibiotik.. hanya painkiller sahaja. demam sampai.... tapi gagah kan jua....

mungkin 3-4 months lagi buat satu lagi op yang terbaring tuh.. takut gak.. sbb yg ni sakit dan doctor tak leh jahit sbb kat dalam.. so dia berhati hati sgt... yg next ni dia terangkan kene jahit...

waaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaaa dugaan dugaan....

wahai rakan seperjuangan... tabahkah laa hati mu

kite baru lepas operation gigi last 2 weeks..skrg dah ok dah gigi ni..alhamdulillah.

merana sakit gigi ni rasanya dekat setahun jugak...sbbnya takut gi dentist.
kedang sakit gigi tu datang menyerang.kekadang dia ok..

tapi baru baru ni asyik keluar nanah je.campur dgn darah.pastu sakit sgt sanpai nak makan apa2 pun x selera.daripada merana dok bela penyakit akhirnye,pegilah kite jumpa dentist.

dentist kata...gigi bongsu U dah tumbuh tapi kedudukan senget dan x cukup space..lebih kurang camtu lah dia kata.

dia check2 tgk..dia kata nanah byk sgt kat gigi tu.mau ka dok bela nanah dalam mulut ? kalo takmo kena laa operate.kalo mau jugak bela penyakit, ok laa..tapi nanti takut infection..

apalagi..kite pun stuju le nak operate.lepas xray,kite appointment utk operate on the next week.Alah..tak sakit pun.sbb dia dah letak ubat kebas.

kena jahit dlm 4/5 jarum kot.

pastu kena makan bubur je 2..3 hari
x leh senyum dan ckp byk..
x boleh lasak2 sgt...

sekarang gigi kite dah ok dah....:clap:

sape2 yg sakit gigi tu cepat2 le gi dentist...jgn lee bela penyakit.

jumpa gak gang kat sini!

saya pun baru 5 lepas hari buat op. 4 batang gigi doktor cabut. Kalau doktor tanye nak ubat penenang ke taknak, baik amekla. Mase saya bedah dulu doktor tak tanye. Menggigil tangan pegang katil bedah.

Lagi satu, baik beli jus epal awal awal sebelum bedah, pasal nanti lepas bedah tak boleh ludah darah keluar (nasihat doktor). Kene telan. Baik telan darah sekali ngan jus epal, baru sedap. Kalau ludah nanti, akan ade komplikasi, pahtu infection, pahtu sakit lagi teruk dari sakit bedah.

Pahtu lagi baik buat mase belajar, atau mase ade health insurans. pasal free. Sebatang tu lebih kurang 300ringgit kot kalau kat malaysia. 4 batang? 1200ringgit. Dah boleh pegi melancong ke pulau tioman selama 2 minggu.

pahtu baik belajar buat bubur yang sedap sedap awal awal. Barula dapat sikit makan mase muka tangah bengkak cam muka doraemon.

oh, lagi satu, jangan takut. Kalau korang bawah umur 25 tahun, memang doktor akan suruh cabut gigi bongsu kalau die tumbuh tak betul. Tapi kalau dah lebih umur 25, dok suruh cabut hanye bile sakit je.

Kalau takut gak, mintak la ubat penenang yang paling power sekali ngan doktor. Terus tido 2 hari straight. Sedar sedar je, gusi dah kurang dah bengkak. Luka dah nak sembuh.

eiiii takutnyer.....kite ada 3 geraham bongsu tgh tumbuh...satu dah fully tumbuh, nasib baik tumbuh betul2..yang 2 lagi baru nak kuar...dulu mmg sakit, but lately dah tak sakit...so kene gi cabut gak ker? first geraham bongsu tumbuh ada space

hmmm...gigi bongsu nak tumbuh...pernah dgr tak salah satu dari tiga kesakitan yg kita akan hadapi...paling tak sakit ialah gigi bongsu nak tumbuh, no. 2 nak branak esp nak first (primip)dan yg paling sakit ialah mati...well aku nak ckp pasal no. 1...actually bengkak sbb gigi bongsu tak semestinya kene cabut,. Royal College of Surgeon England keluarkan puluhan article about this and reasons why extraction should be the last choice...keadaan yg menyakitkan nie berlaku biasanya bila ada halangan bila gigi nak tumbuh tak kiralah sama ada tulang yg halang (disebabkan oleh kekurangan ruang) atau soft tissue (gusi)...tp kalau sakit sbb gigi berlubang lain sakit dia memang lain...so kita cerita sakit masa tumboh...biasanya kesakitan dia akan ada bengkak kekadang radiate ke tmj (temporomadibular joint), eam (external auditory meatus - lubang telinga luar kekadang ke dalam) ada yg teruk sampai ke leher...ada tu siap ada nanah lagi...kes yg paling best aku pernah handle iaalah sakit siap bengkak kat leher siap ada nanah sebesar sebiji bola tenis kat bawah dagu...as i mention above, extraction will be my final choice...so the treatment done at that time is incision and drainage plus intravenous medication...pas seminggu dok dlm ward my patient free to go home...eventhough bacteria is the culprit, yet with modern antibiotic matter can be solve...so before deciding to do any extraction or surgery to the third molar ada beberapa investigation yang perlu dijalankan paling senang visual examination, kalau sakit sebab halangan soft tisu...buang je gisu tu...tak sakit langsung...kalau dgn gomen sku buat free...tp kalau impacted tak kiralah apa anglenye, minor oral surgery will be considered...kalau patient nak....kalau tak adalah regiment ubat yang aku biasa buat dan alhamdulillah mmg berkesan...mmg prbet cas mahal buat MOS...tp kalau gomen rm50..blh mc 3 hari...tak puas hati buleh komplen...banyak lg nak cerita pasal 3rd molar...nntlah...adios amigos....

one of my brother buang his gigi bongsu bulan lepas. Nasibler gigi bongsu dia tu tak dlm keadaan baring or senget.

Lepas cabut doktor ckp tak boleh makan barang panas dan keras. Satu minggu dia makan bubur nasi ~ itupun yang sudah disejukkan terlebih dahulu. Sekarang dah okay - bukan main lagi dia makan benda benda yang dia tak boleh makan dulu

So siapa siapa yg ada prob tu pergi ler jumpa dentis - kot kalau suruh cabut, cabut jer ler dari menahan sakit sepanjang masa kan kan

aku baru jer cabut gigi tadi. gigi bongsu punya pasal la. tp yg pelik, aku sakit gigi belah bawah, tp gigi atas yg dicabut. gigi bongsu tu mcm tak kuar abis. gusi jadi bengkak, cover gigi bongsu tu. siap buleh selak lagi gusi tu. ingatkan nk cabut jer gigi bongsu tu, tp dentist tu ckp kena tunggu gusi surut bengkak. solutinnyer, dia cabut yg atas yg duk elok2 jer tu, utk elak tergigit gusi yg bengkak. aduu...sakit yg amat bila ubat bius tu dh abis tempoh dia... tp alhamdulillah, boleh mamah dh sket2. cuma, risau ni, nak kena cabut jugak ker si bonsu yg punca masalah tu?

wisdom tooth bawak masalah aje sebenarnye. sooner or later mesti remove gak:ah: kalau mude, mase sihat lagi gigi tu, cepat cepatla remove sementare akar die lembut lagi n tak tue.

myself dah 4 surgeries buat:geram: 2 atas 2 bawah. satu surgery 500ringgit.so spent 2k.berturut-turut lak tu lepas baik.

first surgery tu sebab gg rosak.2gigi atas baru tumbuh tu sihat tapi sebab hujung sgt,so sakit n i couldnt bear with the pain.pastu nak gosok pun tak kene...had to remove.lagi 1 gigi tu impacted,ade dalam gusi.

very first surgery tu kat bawah...mase recovery period tu,memang pipi bengkak sminggu.mcm letak baseball dalam mulut.lepas sminggu baru boleh makan solid food n kunyah.if not nak bukak mulut tak boleh sebab sakit.nak telan saliva pun sakit.boleh bukak mulut cukup untuk masukkan straw aje.teruk maybe sebab gigi tu memang rosak n mase nak tarik gigi tu susah.

kalau gigi kite ade masalah..memang recovery period teruk. kalau gg sihat,tak teruk langsung. then also bergantung kepade kepayahan nak tarik gigi tu keluar. kalau senang recovery period tak teruk n vice versa

then,gigi belah atas tu tumbuh kat hujuuuung skali. kalau gosok memang tak kene n tgk cermin memang tak nampak:geram: so lepas 3-4 bulan tumbuh me removed. yang 2 surgeries belah atas tu..tak bengkak langsung.:clap:lepas surgery siap pergi makan kat luar lagi :lol. macam tak cabut gigi langsung.padehalnye buat surgery. the recovery period wasnt bad at all:bgrin: dentist tu kate belah atas memang tak teruk..kate sebab kedudukan die tu atas tulang.n also mude lagi akar tu.so cepat baik.tapi bile die nak buat kerje susah sgt la, sebab hujung sgt.n my mulut ni kecik sangat.so bayangkanla besar mane je boleh bukak n camane doc nak masukkan jari tu.sigh

last wisdom tooth tu was impacted  kedudukan baring:cry: sedih ohh..sebab rase dah tak sanggup dah nak melalui bende yang same.firstly, takut recovery period teruk mcm mule dulu sebab belah bawah.secondly, sebab tak larat kene cucuk dah. tapi i'd to, sebab doc kate nanti dapat infection.yg ni yg teruk. doc kate max 2 bius je...tapi lepas tu rase sakit lagi, doc terpakse bagi last one.so maknenye 3 bius. lepas tu sakit lagi, but bearable la.doc kate takleh kasi dah n mase tu pun nak dekat habis dah. later tu tanye nurse kenape.die kate takut body takleh take it.nape setengah org sampai 6 bius boleh eh? :stp:

yg ni recovery period tak teruk sgt. 2 ari je rase sakit kat situ. n tak bengkak baseball.
so skarang takyah gi for surgery dah.weeee

yg gigi belah atas tu sebab hujung sangat. doc dah informed awal-awal...mungkin bile dah baik nanti, bile minum air..air keluar dari hidung...kalau tu terjadi, kene pergi buat jaw surgery pulak. tapi probability 5% only n luckily it didnt happen

Sakit gusi/gigi

Hi,

Me now tgh sakit gigi, tapi tidak lah teruk sgt sampai pipi bengkak or bernanah. walaubagaimanapun, nie kire teruk gaks dr sakit yg lepas2 sebab dulu gusi bengkak pastu surut ade lah dalam 2-3 kali. Ingatkan pasal tak berus gigi dgn betul ke atau kurang floss ke. Dah try berus gigi bebetul, floss lepas makan, tapi jadik gaks and this time sakit berdenyut2 sket and bile tgk kat cermin, ade cut atau lubang kecil dekat gusi. Mmg bengkak sket la gusi nie sebelah kiri pipi.

I pun dah risau klo infection and lagipun dah mula sakit berdenyut2. Pegilah buat chek-up kat poliklinik di seri kembangan, nice woman doktor, lembut je. Doc cakap gigi bongsu dah tumbuh dalam gusi tapi tak leh nak keluar pasal terhalang dgn gigi sebelumnya tuh la. Kena surgically remove it kat hospital pakar Serdang.

My appointment ialah 14 November nie and lepas baca forum2 korang, mmg gerun rasanya apalagi pk kan the infection and risks. And the worst part is the pain, klo unbearable, matilah nak kena mc lelame pasal i cant afford to take mc too long becos me daily paid. klo mc, no pay la....huhuhuhuhu.Lagi plak, me live alone, jauh dari family, tadak sapa nak jaga....huhuhuhu.

Btw, i tak pasti sgt kedudukan gigi bongsu nie, yg pasti kena bedah utk keluarkan, otherwise akan terus sakit. Nak tanye sesape berpengalaman or doktor kat dalam nie, klo based on my story nie agak2 nye tak serius or serius?  and will be very painful? nanti lepas surgery, bole drive tak?? and klo doktor tuh tak offer utk ksk full anesthetic, kite boleh request ke??

saya dah buang both my wisdom tooth sebelah atas.
mmg tak sakit but kalau boleh jumpa dentist lelaki sbb tenaga dia lagi kuat compared dentist perempuan.
saya buang tak serentak, gap dlm 1 tahun baru yg sebelah kanan sakit.

Wisdom teeth, also known as third molars are the last teeth to erupt. This occurs usually between the ages of 17 and 25. There remains a great deal of controversy regarding whether or not these teeth need to be removed. It is generally suggested that teeth that remain completely buried or un-erupted in a normal position are unlikely to cause harm. However, if these impacted teeth are in an abnormal position (a dentist can show you this on an x-ray), their potential for harm should be assessed.


What is an Impacted Wisdom Tooth ?



A tooth becomes impacted due to lack of space in the dental arch and its eruption is therefore prevented by gum, bone, another tooth or all three. Lack of space occurs because our jaws have become smaller (through evolution), we do not loose teeth through decay as frequently as in the past, and our diet is such that our teeth do not wear down as much.

What are the Indications for Removing Wisdom Teeth ?

Wisdom teeth generally cause problems when they erupt partially through the gum. The most common reasons for removing them are:

Decay

Saliva, bacteria and food particles can collect around an impacted wisdom tooth, causing it, or the next tooth to decay. It is very difficult to remove such decay. Pain and infection will usually follow.

Gum Infection (Pericoronitis)

When a wisdom tooth is partially erupted, food and bacteria collect under the gum causing a local infection. This may result in bad breath, pain, swelling and trismus (inability to open the mouth fully). The infection can spread to involve the cheek and neck. Once the initial episode occurs, each subsequent attack becomes more frequent and more severe.

Pressure Pain

Pain may also come from the pressure of the erupting wisdom tooth against other teeth. In some cases this pressure may cause the erosion of these teeth.

Orthodontic Reasons

Many younger patients have had prolonged orthodontic treatment to straighten teeth. Wisdom teeth may cause movement of teeth (particularly the front teeth) when they try to erupt and this will compromise the orthodontic result.

Prosthetic Reasons

Patients who are to have dentures constructed should have any wisdom tooth removed. If a wisdom tooth erupts beneath a denture it will cause severe irritation and if removed, the patient will need to have a new denture constructed as the shape of the gum will have changed.

Cyst Formation

A cyst (fluid filled sac) can develop from the soft tissue around an impacted wisdom tooth. Cysts cause bone destruction, jaw expansion and displacement or damage to nearby teeth. The removal of the tooth and cyst is necessary to prevent further bone loss. Rarely, tumors may develop within these cysts or the jaw may fracture spontaneously if the cyst grows very large.



Why Should an Impacted Wisdom Tooth be Removed if it Hasn't Caused any Trouble?

Impacted wisdom teeth are almost certain to cause problems if left in place. This is particularly true of the lower wisdom teeth. Such problems may occur suddenly, and often at the most inconvenient times.



When is the Best Time to Have my Wisdom Teeth Removed?

It is now recommended by specialists that impacted wisdom teeth be removed between the ages of 14 and 22 years whether they are causing problems or not. Surgery is technically easier and patients recover much more quickly when they are younger. What is a relatively minor operation at 20 can become quiet difficult in patients over 40. Also the risk of complications increases with age, and the healing process is slower.



Travel to Inaccessible Places

If you are going to an area where specialist dental services are not available and your wisdom teeth are impacted, it may be advisable to have them removed beforehand.



Should a Wisdom Tooth be Removed When an Acute Infection (Pericoronitis) is Present?

Generally, no. Surgery in the presence of infection can cause infection to spread and become more serious. Firstly, the infection must be controlled by local oral hygiene, antibiotics and sometimes the extraction of the opposing wisdom tooth.

The Pro's and Con's of Wisdom Tooth Removal

Some Pro's of Removing a Wisdom Tooth

            * Wisdom teeth may be hard to access with your toothbrush or floss. Over time, the accumulation of bacteria, sugars and acids may cause a cavity to form in the tooth. If it is not restored with a filling, the cavity may spread and destroy more tooth structure causing severe consequences to the tooth and surrounding supportive structures.

            * Due to the difficulty of keeping these teeth clean with your daily home care (brushing and flossing), bacteria and food debris remaining on the wisdom teeth may present a foul smell-causing bad breath.

            * A wisdom tooth that is still under the gums in a horizontal position (rather than a vertical position) may exert pressure to the surrounding teeth, causing crowding and crooked teeth. This also may occur if there is not enough space in the mouth for the wisdom tooth. This may warrant braces to repair the damage.

            * A wisdom tooth that is still under the gums may become irritated. The gum tissue that lays over the tooth may harbor food debris and bacteria that gets trapped under the gum, resulting in an infection in the gums.


Some Con's of Removing the Wisdom Teeth

            * Depending on the size shape and position of the tooth, removal can vary from a simple extraction to a more complex extraction. With a simple extraction, there is usually little swelling, bruising and/or bleeding. More complex extraction will require special treatment which may result in more bruising, swelling and bleeding. However, your dental professional will provide you with post treatment instructions to minimize these side effects.

            * Following an extraction, a condition called "dry socket" may occur. If the blood clot that formed in the extraction area becomes dislodged, it exposes the underlying bone. This condition is very painful, but resolves after a few days. It is preventable by following the post treatment instructions provided by your dental professional.

            * The longer you wait and the older you get, there is the potential for more problems to occur. This is because as you get older, the bone surrounding the tooth becomes more dense, making the tooth more difficult to remove. The healing process may also be slower.

            

Post Operative Care

1. Do Not Disturb the Wound

            In doing so you may invite irritation, infection and/or bleeding. Chew on the opposite side for the first 24 hours.

2. Do Not Smoke for 12 Hours

            Smoking will promote bleeding and interfere with healing.

3. Do Not Spit or Suck Through a Straw

            This will promote bleeding and may dislodge the blood clot, which could result in a dry socket.

4. Control of Bleeding

            If the area is not closed with stitches, a pressure pack made of folded sterile gauze pads will be placed over the socket. It is important that this pack stay in place to control bleeding and to encourage clot formation. The gauze is usually kept in place for 30 minutes. If the bleeding has not stopped once the original pack is removed, place a new gauze pad over the extraction site.

5. Control of Swelling

            After surgery, some swelling is to be expected. This can be controlled through the use of  cold packs, which slow the circulation. A cold pack is usually placed at the site of swelling during the first 24 hours in a cycle of 20 minutes on and 20 minutes off.

            After the first 24 hours, it is advisable to rinse with warm saltwater every two hours to promote healing. (one teaspoon of salt to eight ounces of warm water).

6. Medication for Pain Control

            Anti-inflammatory medication such as Aspirin or Ibuprofen are used to control minor discomfort following oral surgery. Stronger analgesics may be prescribed by the dentist if the patient is in extreme discomfort.

7. Diet and Nutrition

            A soft diet may be prescribed for the patient for a few days following surgery.

            

Following the removal of your wisdom teeth it is important that you call your dentist if any unusual bleeding, swelling or pain occurs. The first 6-8 hours after the extraction are typically the worst, but are manageable with ice packs and non-prescription pain medication. You should also plan to see your dentist approximately one week later to ensure everything is healing well.
            

It is very important to talk to your dentist about extraction procedure, risks, possible complications and outcomes of the removal of these teeth. The actual extraction may be done by a dentist or it may be referred to an oral surgeon, who is a specialist. This decision is based on the dentist抯 preference and the unique features of each individual case. If you are unsure about whether or not to proceed  with the treatment suggested by your dental professional, it is a good idea to get a second opinion. If you decide after consulting with a dentist to not have any teeth extracted, they should be monitored at every dental visit.
